Pre-Admission

You've recently been told by your doctor that you require an inpatient or outpatient hospital procedure or surgery as part of your treatment.

Baptist provides a simple pre-admission program that can help reduce any anxiety you may be experiencing and make everything as smooth as possible on the day of the procedure.

Step 1: Call 968-1400 to schedule a pre-admission session.

Depending on what procedure your doctor has ordered, you may require advance testing such as EKGs, chest x-rays and lab work. When you call 968-1400, Baptist professionals will set up an appointment with the staff in Pre-Admissions to complete all necessary paperwork and tests you may need.

You may wish to call before you leave your doctor's office. In many cases, you will be able to get a pre-admission appointment immediately.

Pre-admission appointments are available Monday through Friday, 7 a.m. to 6:00 p.m.

Step 2: Come to Baptist for your pre-admission session.

When you arrive for your pre-admission session, you will meet with the pre-admissions clerk, who can help you complete all the necessary paperwork for your procedure. You may also submit your paperwork online before your arrival. If you choose to complete paperwork online, you will only have to provide a copy of your insurance cards and sign authorization forms when you meet with the clerk in person. :: Click here to complete an online pre-registration form.

After completing necessary forms, you will then meet with a nurse to review your medical history and complete any necessary tests. The entire process takes approximately one hour.

When you come, be sure to bring:

  • an accurate list of all medications you are currently taking
  • list of any known foods and/or drugs to which you are allergic
  • your insurance card(s)
  • any papers given to you by your doctor
  • a photo ID for positive identification

Step 3: Report directly to Same Day Surgery at the time of your procedure.

When you arrive for your procedure, please park in the parking garage, then report directly to the Same Day Surgery area located on the second floor of Baptist Medical Center, or to another location as otherwise directed by your preadmission nurse.
