Online Access Card Application

Thank you for your interest in Baptist Health Systems' free Baptist Access Card, your key to our simple and effective program for streamlining the admissions process.

When you receive your card, encrypted on it will be your personal medical record number; therefore, no one else can use it. When you present your Access Card at Baptist, the computer will read your card so that your demographic information can be verified for accuracy and your admission completed more efficiently.

To receive your free Access Card, complete the on-line application card. The application must be filled out completely . Please allow three weeks to receive your card.

Before you begin, here are some items you will need to have at hand:

• your Social Security Card
• your insurance card(s)
• your insurer's mailing address for filing claims
