Admissions Checklist

When it's time to go to the hospital to have your baby, the last thing on your mind should be paperwork. To help you avoid last minute admissions issues, here's a checklist of what you should do. This will save time and streamline the admission process when you arrive to have your baby.

Completing this process early in your pregnancy allows you to take advantage of:
• preferred registration in child birth classes
• complimentary membership in Natal Fit water aerobics
• after delivery, complimentary three-month membership to the Baptist Healthplex.

(    ) Complete preadmission application.
Complete The Birthplace pre-admission application available in your physician's office or click here to apply online. Mail or fax (to 601-968-4084) your completed written application along with a photocopy of both sides of your insurance card. A return envelope is provided with the written application. If you complete and submit the online form, it is not necessary to mail or fax any other information.

(    ) Schedule your preadmission appointment.
Call the Baptist Health Line at 601-948-6262 or 1-800-948-6262 to schedule your preadmission appointment.

(    ) Complete your preadmissions appointment.
Baptist's preadmissions is conveniently located in the lower level of Baptist for Women. During this process, the OB admission clerk will provide you with an estimate of the hospital charge. The OB clerk also will need to make a copy of your insurance card and ID. This appointment also provides expectant families with the opportunity to become familiar with Birthplace services and routines and the variety of educational options available.
