The Health Test

Are you facing risks that can affect your chances of living a long and healthy life? The Health Test can help you find out. It starts with an easy to complete, confidential questionnaire. After you have answered all the questions, you will receive your personalized health profile in the mail within 5 business days. Your profile will contain specific information about how you can improve your health and reduce your controllable long-term health risks. There is no charge for this test. This assessment is available for persons aged 18 and older.

Instructions for the Health Test
To take the Health Test, you must request a Personal Identification Number (PIN). The PIN protects your confidential health information. We process requests for PINs Monday through Friday from 8 a.m. to 4:30 p.m. We will process any requests we receive after hours or on weekends the next business day. We will e-mail your PIN to the e-mail address you provide on the PIN request form. After you receive your PIN, you may take the Health Test at any time.
